The mastermind is revealed to be Skye, the teenage son of the group's therapist, Dr. Carol Elliott. Skye has been using his mother's computer and her access to the women's files to coordinate a series of attacks designed to destroy everything his mother cares about. He is aided by Stephanie Fugate, the teenage final girl from Camp Red Lake, who has been groomed by Skye over two years and acts as his accomplice.

In the final confrontation, Lynnette is shot in the head—surviving only because of a titanium skull plate—and Stephanie nearly kills her, but Heather incapacitates Skye with a toilet tank lid, and Lynnette tackles Stephanie to the ground. Rather than killing either of them, Lynnette chooses to let them face trial. The final act begins when Lynnette and Stephanie drive to Dani's ranch, Big Sky Haven Ranch, where they find Dani burning her belongings in a bonfire. Dani is grieving the loss of Michelle, whose water glass has dried up, and she attacks Lynnette, angry about a book Lynnette supposedly wrote. Lynnette convinces Dani that Skye is the mastermind and that Julia, Marilyn, and Heather are in danger. Dani reveals the others are at Camp Red Lake, and the three women drive there together.

At Camp Red Lake, Lynnette is wounded by a bullet that strikes her titanium skull plate. She climbs to the Main Lodge, finds two bodies, and locates Julia hiding in a storage closet. Julia reports that roughly twenty staff members are at a lakeside memorial for Adrienne. They find Dani badly injured on the lawn, and a gas-masked man in tactical gear attacks them with an axe and automatic rifle.

The group retreats into a reinforced panic cabin, where Marilyn calls 911 on a satellite phone. Lynnette escapes through a trapdoor and lures the shooter through the Wellness Barn, where she is cornered. In the hydrotherapy room, she recognizes the shooter's tactical boots and identifies him as Skye. He is about to kill her when Heather appears and strikes him with a toilet tank lid, incapacitating him.
